Ideal Body Weight vs Adjusted Body Weight: What's the Difference?
Ideal body weight (IBW) and adjusted body weight (AdjBW) are two different calculated measurements that are sometimes used in clinical settings. Although their names sound similar, they are not interchangeable.
Ideal body weight is generally an estimated reference weight based mainly on height and, depending on the formula, sex. Adjusted body weight uses ideal body weight together with actual body weight to produce another calculated value. It is particularly relevant to some medication-dosing situations.
What Is Ideal Body Weight?
Ideal body weight is an estimated weight calculated using a mathematical formula. Several equations exist, and the result can vary depending on which formula is selected.
One commonly used clinical equation is the Devine formula:
- Men: 50 kg + 2.3 kg for each inch over 5 feet
- Women: 45.5 kg + 2.3 kg for each inch over 5 feet
For example, using the Devine formula, a man who is 5 feet 10 inches tall has an estimated IBW of:
50 + (2.3 × 10) = 73 kg

What Is Adjusted Body Weight?
Adjusted body weight is a calculated value that uses both ideal body weight and actual body weight.
A commonly used formula is:
AdjBW = IBW + 0.4 × (ABW − IBW)

- ABW = actual body weight
The calculation adds 40% of the difference between actual and ideal body weight to the ideal body weight.

Some drugs may distribute differently into adipose and lean tissues, so using actual body weight, ideal body weight, or an adjusted value can produce different dosing estimates.
Adjusted body weight is not automatically appropriate for every medication; the appropriate body-size descriptor varies by drug and clinical circumstance. (Pai, 2012).

Example: Comparing Ideal and Adjusted Body Weight
Consider an adult with:
- Height: 5 feet 10 inches
- Actual body weight: 100 kg
Using the Devine formula for a man:
IBW = 50 + (2.3 × 10)
IBW = 73 kg
Now calculate adjusted body weight:
AdjBW = 73 + 0.4 × (100 − 73)
First:
100 − 73 = 27 kg
Then:
27 × 0.4 = 10.8 kg
Finally:
73 + 10.8 = 83.8 kg
The three values are:
- Actual body weight: 100 kg
- Ideal body weight: 73 kg
- Adjusted body weight: 83.8 kg
The 100 kg figure is the person's measured weight. The 73 kg figure is an ideal-weight estimate from the selected formula. The 83.8 kg figure is calculated from the other two values.
Ideal Weight vs Actual Weight
Actual body weight is straightforward: it is the person's measured weight at a particular time.
Ideal body weight is different because it is calculated rather than directly measured.
For example, someone may have an actual weight of 100 kg and an estimated ideal body weight of 73 kg. That difference does not automatically mean the person needs to lose 27 kg.
Healthy-weight assessment is broader and can include body composition, waist circumference, physical activity, medical history, and other health indicators.
Adjusted Weight vs Actual Weight
Adjusted body weight should also not be confused with actual weight.
If someone weighs 100 kg, their actual weight is still 100 kg even if an adjusted body weight formula produces 83.8 kg.
In medication-related calculations, a weight descriptor should never be used by itself to change a prescribed dose.
